Applications, Registration and Membership information are available online for download by using the links below. These documents are in Adobe .pdf format and can be printed out or saved on your computer.

How To Download These Documents:
1. Make sure you have Adobe Acrobat Reader installed on your system. If not, simply click on the “Download Acrobat Reader” link above to download it.
2. Click on the title of the desired form. In a new window, the form will open in Acrobat Reader. You can then print the form.
3. If you want to save the form on your computer, right click on the title of the form you want. When prompted, save it to the desired folder on your hard drive.

HOW TO REGISTER YOUR HORSE - Registration Instructions (.doc file)
MEMBERSHIP FORM - Membership application for Lipizzan Association of North America.
REGISTRATION FORM - Application for Registration of full or part-bred Lipizzan horse.
VERIFICATION FORM - Application for transfer of ownership of a previously registered full or part-bred Lipizzan horse.
STALLION REPORT - List of mares bred to a stallion during a single year.
DNA APPLICATION #1 - One of two forms required for DNA testing. This one to be sent to L.A.N.A. representative.
DNA APPLICATION #2 - Second of two forms required for DNA testing. This one to be sent (with fee) to the University of Kentucky.
COLLECTION/INSEMINATION CERTIFICATE - For cooled transported semen or frozen semen.
TRANSPORTED SEMEN/EMBRYO TRANSPLANT BREEDER’S CERTIFICATE - This form is to be used for mares bred with cooled, transported semen and embryo transplant.
EMBRYO TRANSFER DONOR MARE ENROLLMENT FORM - Must be submitted to the LANA office before the collection of an embryo.
EMBRYO TRANSFER RECIPIENT MARE ENROLLMENT FORM - Must be submitted to the LANA office before the collection of an embryo.
